Grounded Path Support provides Independent Support Coordination within the NDIS space, with a focus on life transitions, capacity-building, and steady partnership.
My work is centred around supporting school-aged participants, young people moving into adulthood, and individuals navigating key decision-making periods such as employment exploration and supported accommodation pathways.
This is not crisis-driven coordination.
It is proactive, thoughtful, and grounded in long-term development.
Anchored in reality, steady in approach, and practical in delivery.
Recognising that each person’s journey is individual and evolving.
A partnership rooted in guidance, respect, and shared decision-making.
Grounded Path Support delivers independent coordination that is transition-focused, trauma-aware, and capacity-building in nature. I aim to provide consistent, proactive guidance that helps participants navigate the NDIS with clarity, stability, and long-term direction.
I specialise in supporting key life transitions — particularly school to adulthood, early employment exploration, and decisions around Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA). These periods require thoughtful planning and steady guidance.
My coordination style is mentoring-informed and trauma-aware. I prioritise listening, clarity, and respect. The goal is not simply to organise services, but to support growth, confidence, and informed decision-making.
Rather than crisis-only support, I focus on building capacity over time — strengthening skills, increasing independence, and helping participants navigate the NDIS with greater confidence.
